Additional Information:

• Most in-house labs are included; outside labs, imaging (e.g., X-rays), and specialized send-out tests are billed separately and are not included
• Outside pharmacy charges are not included
• Extended visits or complex procedures may incur additional fees, which will always be disclosed in advance.
