This Notice describes how your medical information may be used and disclosed, and how you can access this information. Please review it carefully.

Eastlake Primary Care is required by law to maintain the privacy of your protected health information (PHI) and provide you with this Notice.

How We May Use and Disclose Your Information

We may use or share your PHI for the following purposes:

Treatment: To provide, coordinate, or manage your healthcare services.
Payment: To obtain reimbursement for services from you, your insurance company, or other payers.
Healthcare Operations: For quality assessment, training, licensing, or administrative purposes.
Required by Law: When disclosure is mandated by federal, state, or local law.
Public Health and Safety: To report disease, injury, or abuse, or to prevent a serious threat to health or safety.
Business Associates: We may share information with trusted vendors (e.g., billing or lab services) under signed Business Associate Agreements (BAAs).

Any other use or disclosure of your PHI will require your written authorization, which you may revoke at any time.

Your Rights Regarding Your Health Information

You have the right to:

Access and obtain a copy of your medical records.
Request amendments to your PHI if you believe it is incorrect or incomplete.
Request restrictions on how we use or disclose your PHI (though we are not required to agree to all requests).
Request confidential communications (e.g., alternative contact methods).
Receive an accounting of disclosures (list of certain non-routine disclosures made).
